Contact form

The Contact form block adds a built‑in contact form for customer messages.

What it does

This block displays:

  • A contact form with name, email, and message fields

Block settings explained

Show placeholders

  • Enabled: placeholders are shown inside the actual inputs
  • When enabled, you can edit the placeholder of custom fields

Labels size

  • Controls the size of all form labels

Labels case

  • Controls the case (uppercase or default) of all form labels

Contact form blocks

Name & email

  • Required fields, will take in merchant's name & email address

Field

  • Customizable text input
  • Can be single line or multiline
  • It can be required or optional

Dropdown

  • Customizable select input
  • Can hold multiple options
  • It can be required or optional

Checkbox

  • Customizable checkbox input
  • It can be required or optional

Form Content

  • Built‑in fields handled by Shopify.

Submit

  • Required button for submitting the form
  • Has regular button styling options

Common use cases

  • Contact pages
  • Support sections

Troubleshooting

  • If submissions don’t arrive, check store email settings

Note: Messages are sent to the store’s configured contact email.
